How to Access Your Medical Records
As a patient at Lynn Community Health Center, you have a right to inspect and/or obtain a copy of your medical records, or have copies of your medical records sent to another facility.
The health center requires a completed and signed Authorization for the Release of Information Form before releasing any documents. In certain cases, a patient’s physician may also be required to approve a request.
Medical records released to individuals must be picked up in person. Medical records released to another facility will be mailed directly to that facility.
To Request a Copy of Your Medical Records
Print and complete the Authorization for Release of Health Information Form.
The form must be completed, signed and dated by the patient or his or her legal representative.
